Welcome to the website, mobile application, related sites, and online and digital services of ACCURACY RESEARCH (the “ACCURACY RESEARCH Site,” the “Site,” or the “Sites”), an e-commerce, information, and online communication service provided by ACCURACY RESEARCH and all its affiliates (“ACCURACY RESEARCH” or “We”).

What Information We Collect

Information You Provide to Us: We respect the privacy rights of all visitors to ACCURACY RESEARCH Sites. We receive and store certain information that you enter on our Sites or provide to us in any other way through them.

Information you provide when completing forms on our Sites, such as forms for requesting medical appointments or product orders. This also includes information you provide when registering to use our Sites, purchasing products, or using services available on the Sites or at centers. We may also ask you for information when you report a problem with our Sites. Some forms collect sensitive information, such as medical information, necessary to provide you with our services;

Records and copies of your correspondence (including email addresses) if you contact us;

Your responses to surveys we may ask you to complete for research, development, and marketing purposes; and

Details of transactions you carry out through our Sites and the fulfillment of your orders. You may be required to provide financial information before placing an order through our Sites.

Information We Collect Automatically: We automatically gather information about you as you navigate or use our Sites. The information collected automatically may include usage details, IP addresses, session replay technology, and recording (tracking your movements, clicks, etc., on our Sites), and information collected through cookies, web beacons, pixel tags, and other tracking technologies. Location Data: Information about your location collected through geolocation technology. ACCURACY RESEARCH applications and services operating on mobile devices typically do not use geolocation. However, the ACCURACY RESEARCH application may collect and use geolocation information solely for the limited purpose of facilitating check-in to your scheduled medical appointments upon arrival. You can disable this feature on your mobile device at any time by revoking the authorization allowing the ACCURACY RESEARCH application to use location services.

The information we collect automatically may include Personal Data or we may retain or associate the information we collect with Personal Data that we obtain in other ways or receive from third parties. This helps us improve our Sites and provide a better and more personalized service by enabling us to:

  • Estimate the size and usage patterns of our audience;
  • Improve our product and service offerings;
  • Store information about your preferences, allowing us to customize our Sites according to your individual interests; and
  • Recognize or authenticate you when you return to our Sites.

Our Sites offer you the opportunity to receive messages from us or third parties. For example, you can subscribe to a free email newsletter from ACCURACY RESEARCH. You can unsubscribe from this newsletter at any time. You can use the opt-out mechanism or unsubscribe link provided in our promotional emails if you wish to stop receiving such emails from us.

Emails you send to us through the links on our Site may be shared with a customer service representative, employee, medical expert, or agent who may be better able to respond to your concern. We strive to respond promptly after receiving communication. Once we have responded to your message, it will be deleted or archived, depending on the nature of the concern. We will retain your information as long as your account is active or as needed to provide you with services, comply with our legal obligations, resolve disputes, and enforce our agreements. However, we may not be able to fulfill that request in all cases, depending on the laws and regulations governing us. For example, under federal and state laws, we may be required to retain specific identifying information, such as your medical records or other information. Your Rights Regarding Your Information, and Access and Correction of Information

For non-patient information, you can contact us through the contact information provided below to inquire about what information we have about you and to correct it. Additionally, you can review and modify your Personal Data. To do so, you need to log in to our site and visit the Account Settings or Preferences section. You may also notify us through the contact information provided below of any changes or errors in the Personal Data we have about you to ensure that it is as complete, accurate, and up-to-date as possible or to delete your account. We cannot completely delete your personal information unless we also delete the account you have with us. We may not be able to comply with your request if we believe it would violate any law or legal requirement, if we have a legal basis or obligation to retain it, or if it would cause errors in the information.

If you are a patient of ACCURACY RESEARCH, you can access the patient-related information we have about you in the Internet Services for Patients or our Medical Information Management Department. You can also request a correction to your patient information through our Internet Services for Patients or the Medical Information Management Department. In our mobile app, you can choose to deactivate your account; however, we still have an obligation to retain the patient-related information we have about you under the Health Insurance Portability and Accountability Act and other federal and state laws.

Security

Whether you are visiting the ACCURACY RESEARCH Site or one of our clinics, we use reasonable security measures to protect the confidentiality of personal information under our control and appropriately limit access to it. We use various information security measures to protect your online transactions with us. The ACCURACY RESEARCH Site uses encryption technology, such as Secure Sockets Layer (SSL), to protect your personal information during data transmission. Aside from our Internet Services for Patients, our Sites are not intended for users under the age of 18. No one under 18 may provide information on the Sites or through them. We do not knowingly collect Personal Data from users under 18. If you are under 18, do not use or provide any information on our Sites, including your name, address, phone number, email address, or any screen name or username you may use. If we learn that we have collected or received Personal Data from a user under 18 without parental consent verification, we will delete it. Our Internet Services for Patients are available to patients aged 18 and over. They are also available to patients aged 13 to 17 with parental or legal guardian permission.
